Indications TAULIZ ® Piretanide

TAULIZ ® is indicated for the treatment of high blood pressure and oedematous conditions of cardiac, hepatic and renal genesis.

Mechanism of action TAULIZ ® Piretanide

The active ingredient of TAULIZ ® the piretanide, after oral administration is effectively absorbed through the intestine, with a plasma peak between the first and the second hour. After having exercised its diuretic action on the kidney - inhibiting the reabsorption of chlorine and sodium, and to a lesser extent of potassium, in the ascending tract of the loop of Henle (portion of the nephron) - it is eliminated mainly at the renal level, and in small part through the feces, after about an hour and a half. Naturally the kinetics of intravenous administration is much faster.

The increased excretion of ions and the inhibition of their reabsorption promotes an important loss of water, which contributes to the improvement of hypertensive and edematous symptoms.

Studies carried out and clinical efficacy

1 DUCK DIURETICS AND HYPERTENSION

One of the most common therapeutic indications for loop diuretics, including piretanide, is arterial hypertension. A careful critical reading of the literature has shown that the best effect of these diuretics, and in particular of the piretanide taken at 3 / 6mg doses, has been a reduction of the pressure levels of 8mmHg for the maximum and 4mmHg for the minimum.

2. THE EFFECTS OF COMBINED HYPERTENSION THERAPY

In this study conducted on 611 patients with hypertension, the therapeutic efficacy of the combination of piretanide, active ingredient of TAULIZ ® with other antihypertensives (ACE inhibitors) was evaluated. Compared to monotherapy, that is therapy with a single product, the combined therapy was able to guarantee a further lowering of the maximum and minimum pressure levels of about 2 mmHg, thus making the therapy more effective, without side effects worthy of note.

3. DUCK DIETES AND ASCITES

In this study the efficacy of some diuretics and antihypertensives in the treatment of ascites due to liver cirrhosis was evaluated. The data show that the most effective and safe of the diuretics of the loop is furosemide and piretanide, although the best effects in the treatment of this pathology have been observed with antialdosteronics. However, combined therapy appears to be the best possible application.

Method of use and dosage

TAULIZ ® 3/6/9/12 mg tablets of piretanide: it is recommended to take about 6 mg in a single dose, better if in the morning, in the treatment of the edematous condition.

Under the strict medical supervision, in case of ineffectiveness of the therapy, it is possible to increase the dose of 3/6 mg, taking it at intervals of at least 4 hours.

In the treatment of high blood pressure, the most effective dose appears to be that of 9 mg / day taken in a single dose or fractionated during the day.

TAULIZ ® Piretanide warnings

Before and during treatment, especially if prolonged, with TAULIZ ® it is necessary to carefully evaluate the serum sodium, potassium and chlorine levels, and correct them if necessary.

The increased excretion of potassium that is obtained following the administration of TAULIZ ® exposes the patient to potential hypokalemia, especially in the case of diarrhea, vomiting, use of laxatives and mineralocorticoids; therefore it would be appropriate to combine pharmacological therapy with a diet rich in potassium and possibly also with appropriate integration protocols.

The phenomenon of haemoconcentration, due to the uncontrolled diuretic effect, could determine an increase in uricemia with consequent gouty attacks, while the insulin-resistant effect of sulfonamide diuretics exposes the diabetic patient to potential hyperglycemia, thus making periodic checks of glycaemia and glycosuria necessary. .

Although no direct correlations have been found between the administration of TAULIZ ® of interferences on driving skills or use of machinery, it is always important to consider that an inadequate dose of this drug could lead to hypotensive crises, hyponatremia and hypokalemia, accompanied by a significant reduction in perceptive and reactive abilities of the patient.

PREGNANCY AND BREASTFEEDING

Despite the active ingredient of TAULIZ ® piretanide, which was found to be devoid of a genotoxic and teratogenic effect on animal guinea pigs, the use of this drug is not recommended during the first trimester of pregnancy, limiting its use to cases of real need during the continuation of gestation.

Furthermore, piretanide is excreted in breast milk, so it would be advisable to stop breast-feeding while taking TAULIZ ®

Interactions

TAULIZ ® can lead to a reduction in renal clearance, with a consequent increase in the toxic potential of both lithium salts and digoxin.

It can also interact with:

- other antihypertensives, strengthening their effect and putting the patient at risk of hypotensive crises. In these cases it is necessary to adjust the dosages;

- tubocuranine, with increased effects;

- NSAIDs and other drugs with renal tubular secretion, reducing the diuretic effect of TAULIZ ®

TAULIZ ® Piretanide Contraindications

TAULIZ ® should not be administered in case of renal insufficiency characterized by oligo-anuria, poisoning and poisoning by nephrotoxic and hepatotoxic substances, in case of hepatic precoma and digital overdosage.

As mentioned above, before taking TAULIZ ® it is important to check the electrolyte balance, and possibly correct it if you take the drug.

Undesirable effects - Side effects

The diuretic and saluretic effect of TAULIZ ® especially when the treatment is prolonged for long periods or the drug taken at high doses - can induce cases of hypotension, hypokalaemia, hyponatremia, with consequent dry mouth, tiredness, dizziness, thirst, muscle cramps, micturition disorders and in severe cases ototoxicity and heart rhythm disorders.

TAULIZ ® can also, following hemoconcentration, be responsible for hyperuricemia and gout attacks, as well as alteration of the glycemic profile (especially in diabetic patients).

In case of hypersensitivity to one of the components of TAULIZ ® it is possible to observe skin rashes, vasculitis, paresthesia, photosensitization.

TAULIZ ® should always be used after consulting your doctor.

The indiscriminate use of TAULIZ ® between athletes and non-athletes, in the search for the loss of a few pounds, exposes the body to serious side effects. Furthermore, it is always advisable to reiterate that weight loss is dictated by the elimination of liquids and salts and not by a real weight loss effect, understood as a loss of fat mass.

Therefore TAULIZ ® is classified as a DOPANTI substance.
