Definition

The Spitz nevus is a benign tumor of the skin, which develops mainly during childhood. This acquired lesion is due to the proliferation of epithelioid and fused melanocytes, in many cases atypical.

The Spitz nevus is a melanocytic tumor characterized by a benign clinical course (the transformation in the malignant sense is very rare). This lesion occurs above all in the face, trunk or lower limbs, presenting itself as a uniformly pigmented pink or red-brownish, cupoliform or nodular macula.

Initially, the Spitz nevus grows rather quickly, then the dimensions become stable over time: usually, the lesion is symmetrical, well circumscribed, with a diameter less than one centimeter.

The Spitz nevus can present a histological aspect similar to that of melanoma, with respect to which the differential diagnosis must be placed. Furthermore, during dermatological assessment, the lesion must be distinguished from common nevi, contagious molluscum, pyogenic granuloma, vulgar wart and dermatofibroma. For this purpose, when the Spitz nevus with atypical characteristics reaches a diameter greater than one centimeter and presents asymmetry or ulceration, it must be subjected to biopsy and, sometimes, can be excised completely.

If the patient has small stable injuries, a periodic medical evaluation is indicated, so that changes can be monitored.
