Definition

"Tinnitus" is an annoying ringing or noise, continuous or intermittent, to the ears; we cannot speak of a real disease, rather tinnitus is a common symptom of different diseases or stages of age. For example, very many elderly people complain of tinnitus, a reflection of the physiological loss of hearing due to age or auricular lesions.

Causes

Some diseases can trigger or exacerbate tinnitus: more often than not, it is not possible to identify a single cause. In general, the factors most involved are: atherosclerosis, damage to the inner ear, ear wax deposition, frequent exposure to loud noises, advanced age, hypertension, lesions of the auditory nerves or of the brain, acoustic neuroma, otosclerosis ( stiffening of the bone in the middle ear), otitis, pathology of Meniere, head trauma, tumors of the head and neck.

  • Even the abuse of some drugs can promote the onset of tinnitus: some antibiotics, antimalarials, aspirin (overdose), chemotherapy, diuretics ...

Symptoms

Tinnitus begins with an annoying perception of a noise, similar to a buzzing in the ears, in the absence of external sounds. It is a sort of phantom noise, perceived exclusively by the subject: the noise can be more or less intense, hissing or roaring, continuous or intermittent. Sometimes, the perceived noise is so violent and acute that it even interferes with normal daily activities and concentration.

To treat tinnitus, the doctor will first need to identify the cause that caused the disorder; therefore we proceed with the treatment of the underlying pathology, making use also of parallel therapeutic measures aimed at lightening the perceived noise caused by tinnitus.

In general, the first therapeutic measure undertaken is the removal of earwax plugs formed inside the ear; alternatively, it is possible to treat a vascular alteration (responsible for tinnitus), by administering drugs or targeted surgical procedures.

Clearly, when the cause of the tinnitus depends on a drug, it is advisable to replace it with an active ingredient of similar therapeutic activity.

Some drugs can also help the patient to lighten the ears, although they can develop serious side effects: it is possible to take tricyclic and benzodiazepine drugs, to be administered exclusively for the treatment of severe tinnitus, subject to medical prescription.

If even the drugs could not exercise their therapeutic effects, it is possible to subject the patient to non-pharmacological treatments: we are talking about the application of electrical devices useful to suppress the perceived noise or hum.

Tricyclic drugs : used in therapy for treating tinnitus successfully: do not forget that the administration of these active ingredients can cause serious side effects, such as cardiac dysfunctions, blurred vision, dry mouth, constipation.

  • Amitriptyline (eg Laroxyl, Triptizol, Adepril): although approximate, it is not possible to report a dosage of the drug: it is a medical competence, in fact, to establish the modality of drug intake, the duration of therapy and the dosage, based on the patient's age and the severity of the condition.
  • Nortriptyline (eg Dominans, Noritren): the recommended dose to take the drug varies from a minimum of 25 to a maximum of 150 mg a day; also in this case, the exact dosage must be established by the doctor.

Benzodiazepines : the administration of these drugs can definitely reduce the annoying buzzing perceived in the context of severe tinnitus but, even in this case, the side effects should not be forgotten (nausea, drowsiness).

  • Alprazolam (eg. Xanax, Frontal, Alprazig): based on the cause causing the tinnitus, it is possible to take this drug at a dose variable from 0.25 to 0.5 mg, to be taken orally, three times a day. It is possible to gradually increase the dose, every 3-4 days, without exceeding 4 mg / day. The drug can be addictive: take only after prescription. Do not exceed the dose indicated by your doctor.

Antihypertensive drugs : sometimes, tinnitus is the consequence of an alteration in blood pressure values, therefore the administration of specific drugs, in addition to regulating the blood pressure, removes the ringing in the ears.

Medicines for the treatment of otitis: otitis is an infectious inflammation of the inner ear. In such circumstances, the patient often complains of tinnitus and ear pain on touch; it follows that the administration of antibiotic drugs (to remove the pathogen) and painkillers (to control pain) promotes the healing of the disease (otitis) with all its symptoms (including, of course, tinnitus).
