Indications PERIOSTAT ® Doxycycline

PERIOSTAT ® is indicated in the treatment of adult periodontitis as a pharmacological support for dental hygiene and various dental practices.

Mechanism of action PERIOSTAT ® Doxycycline

PERIOSTAT ® is a drug based on doxycycline, an antibiotic belonging to the second-generation semi-synthetic tetracycline category, particularly known for its wide spectrum of action and for its high therapeutic efficacy supported by particularly advantageous pharmacokinetic properties.

In the dental field, however, doxycycline is used in the treatment of periodontitis not as an antibiotic but above all as a drug capable of inhibiting the activity of metalloproteinases and of the enzyme involved in the synthesis of inflammatory mediators, such as TNF alpha, responsible for the inflammatory lesions present in various pathologies, and of the collagenases present in the gingival fluid of adult patients suffering from periodontitis, able to promote harmful events affecting the gums and dental roots.

The doses of doxycycline present in PERIOSTAT ® and generally used in the dental field in fact do not in any way allow the active ingredient to carry out its own bacteriostatic and bactericidal activity.

Despite the dosages more than halved compared to the common antibiotic therapy, the pharmacokinetic properties of doxycycline remain unchanged, thus maintaining a high bioavailability, a lasting half-life and a predominantly renal excretion.

Method of use and dosage

PERIOSTAT ®

Doxycycline 20 mg coated tablets.

The common therapeutic scheme envisaged for the treatment of adult periodontitis is based on the intake of two PERIOSTAT ® tablets per day divided into two different administrations, preferably at least an hour before meals or an hour before bedtime.

The duration of treatment should not be less than three months.

In any case the supervision of your doctor or dentist during treatment is strongly necessary.

Warnings PERIOSTAT ® Doxycycline

Therapy with PERIOSTAT ® should be supervised by your doctor and your dentist, in order to preserve the therapeutic efficacy of the drug, reducing the appearance of side effects as much as possible.

Closer monitoring should be expected in elderly patients or those suffering from kidney disease.

Prolonged use of tetracycline may facilitate intestinal colonization by microorganisms such as Clostridium difficile, responsible for serious diseases including pseudo-membranous colitis.

The use of tetracycline exposes the patient to the risk of burns, erythema and dermatological reactions following exposure to ultraviolet rays, due to the photosensitivity induced by the antibiotic.

PERIOSTAT ® contains lactose therefore maximum caution is recommended in patients with lactase enzyme deficiency, glucose-galactose malabsorption and galactose intolerance.

PREGNANCY AND BREASTFEEDING

The use of tetracyclines, especially when prolonged over time, is generally contraindicated during pregnancy and in the subsequent period of breastfeeding, given the absence of studies able to characterize the safety profile of doxyliccin on the fetus and infant.

Interactions

In order to preserve the pharmacokinetic characteristics and therapeutic efficacy of doxycycline, while at the same time reducing the risk of potential side effects, particular caution should be exercised in the contextual intake of PERIOSTAT ® and:

  • Oral anticoagulants, given the May risk of bleeding induced by antibiotic therapy;
  • Antacid preparations based on bivalent ions such as magnesium, calcium and aluminum, able to reduce the intestinal absorption of doxycycline;
  • Penicillin and beta-lactams, given the reduction in demonstrated therapeutic efficacy;
  • Oral contraceptives, due to their reduced intestinal absorption and the loss of contraceptive efficacy.

Contraindications PERIOSTAT ® Doxycycline

The use of PERIOSTAT ® is contraindicated in patients with hypersensitivity to tetracycline and its excipients and in patients under the age of 12 years.

Undesirable effects - Side effects

Treatment with doxycycline, despite the low doses used in the dental field, could determine the appearance of adverse reactions such as nausea, vomiting, diarrhea, glossitis, headache, paresthesia, dizziness, anemia, neutropenia, eosinophilia, dermatological reactions such as urticaria, erythema and edema, which fortunately regress readily once therapy is suspended.

Prolonged use of tetracycline, as prescribed for the treatment of periodontitis, could determine an increase in blood urea and plasma creatinine due to the catabolic action of the drug, dental discolouration and enamel hypoplasia in children.
