Hydroxy beta methyl butyrate - little present in food, HMB is synthesized in our body starting from the essential amino acid leucine, for an average quantity of about 200 / 400mg a day. Not yet completely clear is the pharmacokinetic and pharmacodynamic profile of this compound, although for about 15 years it has been experimenting in various sectors, including sports. Given the low quantity produced endogenously, and given the studies reporting ergogenic effects at about 3 gr / day, the best source of intake remains the oral supplement, for which there is a high bioavailability. In fact, at 1 gram of HMB, only 14% of the product is eliminated with urine, while this quantity rises to 29% if taken at a dose of 3 grams. Generally the blood peak is reached 2 hours after its ingestion, with a return to baseline after 9 hours. However, the biodistribution of the molecule remains unclear, which represents a limitation in the development of optimal supplementary protocols. The biological effect, on the other hand, translates into the regulation of protein turnover, with an increase in protein synthesis and reduction of degradation processes, with a decline in apoptotic phenomena and with a potential protection from oxidative damage.

For these reasons the HMB has been successfully used in counteracting cachexia in patients suffering from AIDS or from oncological pathologies, for which in addition to the lack of reduction, an increase in muscle mass has been recorded. HMB has also been shown to be effective in reducing wasting in elderly individuals and in preventing the cytotoxic effects of lipopolysaccharide. Further experiments are also ascertaining the possibility of improving the lipidemic profile, decreasing the circulating levels of LDL (possible application in cardiovascular diseases) and counteracting tumor proliferation.

Given the effects of body composition distribution observed in cachectic patients, a potential use in sports was thought with the intent of:

  • Increase muscle mass (increase in protein synthesis and reduction of degradation);
  • Improve athletic performance (strength and endurance);
  • Protect the muscle from damage induced by intense exercise;
  • Counteract the feeling of fatigue.

Numerous studies have tested the potential of this supplement, using 3 grams per day as a standard dosage and recording mixed results. Despite this - although it is still difficult to draw definitive conclusions and to investigate the biological and pharmacodynamic role of this molecule - from the various studies there are some results on some aspects:

  1. Increased strength in untrained subjects subjected to integration (the question remains on the variability of response to training, given that we start from untrained subjects);
  2. Reduction of markers of muscle damage (Creatine kinase and lactic dehydrogenase).
  3. Ineffectiveness of acute supplementation: there is no positive effect, following administration of HMB limited to one week or less;
  4. No effect on the increase in lean mass in trained athletes.

Possible ergogenic effects on trained athletes or individuals are still very uncertain. It should be noted that unlike other supplements, in this case there are experimental studies, some of which have beneficial effects.

The recommended dosage, and most used, is that of 3 grams per day divided throughout the day.

Pharmacokinetic studies of the compound define the optimal dose at 38 mg / kg, defining doses of less than 1 gram as those with reduced clearance.

Supplementation should be prolonged for at least 3/4 weeks, and many define it as indicated in the start-up phase for sporting activity .

Given the dose of 750 mg per capsule, a supplementary protocol could include taking a tablet in the morning, a pre-workout, a post-work-out and finally a bedtime.

Animal studies also show how very high doses are tolerated.
