Symptoms Cystopielitis

Definition

Cystopielitis is an inflammation that affects both the bladder and the renal pelvis. This inflammatory process can arise in an acute way, as a syndrome in itself, or represent a complication of various diseases affecting the urinary tract.

In most cases, cystopielitis is caused by infections due to pathogens that usually attack the urinary tract (especially Escherichia coli ) and rise from the urethra to the bladder. The bacterium infection can also extend to the kidney (cystopielonephritis).

Furthermore, in debilitated patients with chronic diseases or in immunodepressive therapy, the responsible microorganisms can reach the bladder via a hematogenous route.

The risk factors that predispose to the development of cystopielitis include urinary abnormalities, elderly and pediatric age, presence of urinary tract obstruction, frequent sexual intercourse, diabetes, immunodepression and pregnancy. In men, the disease can also be favored by prostatitis and prostatic hypertrophy.

The onset of the disease is usually rapid. Cystopielitis is characterized by continuous severe pain in the lumbar region, with irradiation towards the inguinal region, and a sensation of weight in the lower abdomen, often associated with dysuria, urinary urgency and pollakiuria.

During the course of the disease, other symptoms occur, such as fever with chills and sweating, a general sense of prostration and pus emission, elements of flaking and / or bacteria with urine (bacteriuria and / or pyuria). Depending on the severity of the infection, it is possible that anorexia, nausea and vomiting are present.

If neglected, cystopielitis can lead to serious repercussions due to the extension of inflammation to the renal parenchyma (pyelonephritis).

The therapy involves the administration of specific antibiotics, the choice of which is guided by the results of the bacterial culture of urine and blood. In addition to these drugs, absolute rest and the use of antispasmodics and analgesics may be indicated.
