Why can the itch remain after scabicidal therapy?

At the end of the treatment with the scabicide drug, the skin irritation may persist for some time, with the difference that the itching no longer occurs at night (as is typical during the course of scabies).

The scabicide kills the mite, but the residues of the parasite's body remain in the thickness of the epidermis and continue to create an allergic stimulus, which is usually exhausted after about two weeks.

Once the scabies has been treated, it may still be useful to relieve the use of emollients, to apply topical corticosteroids or to take antihistamines by mouth to relieve itching.

In some cases, persistent itching depends on therapy failure . This may be due to failure to adhere to or incorrect treatment, reinfestation (or relapse) or resistance to therapy. It is therefore necessary, on the advice of the treating physician, to perform another course of therapy or to change the scabicide product.
