Definition

Painful ejaculation is a rare sexual dysfunction. It is a pain or a burning sensation felt simultaneously or immediately after ejaculation, which is localized at the level of the perineum (area between the anus and the genitals), urethra (channel that runs in the penis and allows the passage of the 'urine from the bladder outside) or from the balano-meatal area (glans and urethral outlet, at the distal end of the penis). Sometimes the painful sensation radiates to the pelvis and testicles. Furthermore, it can interfere with sexual pleasure.

This symptom can be caused by an inflammation or an infection of the organs involved in the ejaculation or of the surrounding structures. Most often, painful ejaculation occurs during prostatitis or urethritis and is often associated with painful urination, stenosis of the urethral canal and obstruction of ejaculatory ducts.

Painful ejaculation can also be seen in prostate cancer, epididymitis, benign prostatic hyperplasia, orchitis and sexually transmitted diseases. Furthermore, it is not excluded that this manifestation may derive from psychological disorders (somatiform syndromes).

Painful ejaculation can be more or less distinguishable from an orgasmic pain, which generally recognizes a neurological etiology.
