Iridodonesi - Causes and Symptoms

Definition

Iridodonesi is a flickering iris that appears when the eye moves. This symptom is due to the lack of support from the crystalline lens, against which, normally, the iris rests, which consequently can oscillate in the aqueous humor.

Iridodonesi occurs in the absence of the lens or its displacement with respect to the normal position (ectopia lentis). Partial dislocation is called subluxation and can occur due to rupture of the suspensory ligament (Zinn's zonula). A dislocation of the crystalline lens, on the other hand, is a complete displacement with respect to its natural location, in the anterior chamber and in the vitreous.

In addition to iridodonesi, the dislocation of the crystalline lens can also involve monocular diplopia, greater depth of the anterior chamber and modifications of the eye's reflective power (which can become hyperopic or myopic).

The dislocation of the crystalline lens can be spontaneous (congenital and often associated with other changes in the eye, such as aniridia, microphthalmos and colobomas) or secondary to post-inflammatory processes or to strong trauma to the eyeball.

Congenital iridodonese is one of the symptoms of Marfan syndrome and Ehler-Danlos syndrome. A movement of the fluctuating iris can also occur in homocystinuria, a rare metabolic disease that leads to an accumulation of homocysteine ​​and its homocystine dimer, with adverse effects including thrombotic tendency, lens dislocation and skeletal anomalies.

Iridodonesi can sometimes be observed as a result of the removal of the opacified lens (cataract).
