Definition

Anal pain is a symptom that recognizes several causes.

If the pain is discontinuous and begins during or immediately after defecation, a proctological examination could highlight the presence of an anal fissure. In this case, the pain manifests itself in the form of rather intense pangs; typical is also the loss of blood in the form of red traces in toilet paper.

If the anal pain is constant and accompanied by the appearance of a small swelling in the anal area, it could be hemorrhoids.

A perianal abscess also manifests as a swelling associated with anal pain. In this case, however, the swelling is hot and extremely painful to the touch (hemorrhoids, on the other hand, except in the most serious and complicated cases, are not particularly painful).

When anal pain occurs without reason in the form of a spasm and after about ten minutes it resolves spontaneously, it could be a spasm of the sphincter muscle.

Less frequent causes of anal pain can be inflammatory bowel diseases (ulcerative colitis or Crohn's disease), anal neoplasms, syphilis and coccygodynia.

In some cases, chronic anal pain may be idiopathic. Factors that can contribute to the problem are previous anal surgery and difficult defecation.
