Indications TOSTREX ® - Testosterone

TOSTREX ® is a testosterone-based gel useful in the treatment of symptoms associated with androgen deficiency.

Mechanism of action TOSTREX ® - Testosterone

TOSTREX ® is a drug based on testosterone, which makes its intake particularly easy, being formulated in gel for skin applications.

Generally applied on the intact skin of the arms or abdomen, the testosterone contained in TOSTREX ® is absorbed at the skin level in the following 24 hours, and bound to plasma proteins such as the Sex Hormon Binding Protein, distributed to various tissues, where interacting with specific receptors nuclear, it is able to modulate the gene expression and the activity of these cells.

More precisely this hormone is acting at various levels ensuring:

  • Increased protein synthesis and muscle masses;
  • Increased calcium absorption and osteodeposition phenomena, therefore being indicated for the prevention of osteoporosis in high-risk patients;
  • Induction of hematopoiesis with a significant improvement in blood concentrations of hemoglobin and blood count;
  • Development of secondary sexual characteristics;
  • Improved sexual function.

After its action, testosterone is eliminated following glucuronation mainly through the urine.

Studies carried out and clinical efficacy

1. TESTOSTERONE AND ENDOTHELIAL DYSFUNCTION

Study demonstrating how endothelial dysfunction present in patients with late-onset hypogonadism and erectile dysfunction may be significantly improved with the use of androgens, reducing the invasiveness of therapy through gel formulations.

2. ANDROGENIC THERAPY IN WOMEN

Work that evaluates the possibility of expanding hormone therapy replacing testosterone with women suffering from menopause and sexual or HIV disorders. At the moment the topical application of this hormone seems to be the easiest therapeutic path to follow in these patients.

3. THE ECONOMIC IMPACT OF TESTOSTERONE-BASED GELS

Very interesting study on the economic impact of the marketing of a testosterone-based product, which significantly reduces the invasiveness of hormone replacement therapy for patients suffering from hypogonadism.

Method of use and dosage

TOSTREX ®

Gel containing 20 mg of testosterone per gram. :

the dispenser in TOSTREX ® allows 10 mg to be released under pressure.

In most cases the maximum therapeutic efficacy is obtained by applying 60 mg of testosterone, equal to 6 pressures, to be performed every morning.

It is clear that this gel must be applied after bathing or showering.

It is always useful to remember that the dosage and the period of application must be defined by the doctor after a careful visit.

Warnings TOSTREX ® - Testosterone

Testosterone intake, even when performed through gel, must necessarily be preceded by a medical examination necessary to assess the prescriptive appropriateness and the possible presence of conditions incompatible with this therapy.

In fact patients suffering from hypertension, epilepsy, cardiovascular, hepatic, renal and metabolic pathologies, prostatic pathologies and familiarity for malignant pathologies should be subjected to routine and periodic medical checks, useful for evaluating the therapeutic efficacy and the possible appearance of signs and symptoms indicative.

Consequently, upon the occurrence of the first signs of imbalance both clinically and psychologically and physically, the doctor should immediately stop the therapy.

The patient must also resort to appropriate hygiene and behavioral rules such as washing hands with soap and water after applying the gel, covering the surface on which the gel was applied with clothing, to avoid testosterone transfer, by contact direct cutaneous or with clothes, to family or similar.

The use in prepubera age should instead take place with particular attention, given the possible deleterious effects on bone growth and on the too rapid development of the male genital organs.

PREGNANCY AND BREASTFEEDING

TOSTREX ® is indicated for the treatment of male diseases.

Testosterone taken during pregnancy, permeating the hematoplacental barrier, could reach the fetal circulation inducing virilization of the fetus and compromising the reproductive capacity of the unborn child.

Interactions

Testosterone intake, even if performed topically, could increase the risk of:

  • Haemorrhages, in patients subjected to simultaneous intake of anticoagulants;
  • Hypoglycemia, in diabetic individuals with ongoing hypoglycemic-based therapy;
  • Edema and hypertension, in patients with cortisone-based therapy.

It is useful to remember that testosterone intake could offset some laboratory parameters indicative of thyroid, renal, adrenal, hepatic and hematological function.

Testosterone intake is contraindicated in case of hypersensitivity to the active substance or to one of its excipients, breast cancer, estrogen-dependent neoplasms, cancer and prostatic hypertrophy, history of hepatocarcinoma, hypercalcaemia, nephrotic syndrome, pregnancy and breastfeeding.

The patch should not be applied to non-intact skin or to skin areas under high pressure.

Undesirable effects - Side effects

Although topical administration of testosterone significantly reduces the invasiveness of the therapy, the application of TOSTREX ® does not spare the patient from the side effects described in any other form of hormone replacement therapy with testosterone.

The side effects, significantly increased in incidence and intensity in doping, involve various systems with the increased risk of developing: prostatic hypertrophy and prostate cancer, weight gain and water retention, hypertension and cardiovascular complications, hypercholesterolemia, acne, hirsutism, changes in liver function, oligozoospermia, alopecia, gynecomastia, dizziness, hypercalcemia, hypercalciuria and myalgia.
