Currently, there are only two drugs approved for use for weight loss: sibutramine and orlistat tablets.

Sibutramine stimulates the sense of satiety, reducing food consumption; it also increases thermogenesis with mechanisms of action similar to those of ephedrine, an active ingredient of natural origin useful for weight loss but no longer marketable as a supplement. The main side effects are of a cardiovascular nature (tachycardia, palpitations, hypertension), such as to make these tablets contraindicated in the presence of cardiac and cardiovascular problems (high blood pressure, atherosclerosis, coronary heart disease, etc.). On 24 January 2010 the sibutramine was again banned from the market, as the potential risks deriving from its recruitment would exceed the possible benefits.

Orlistat-based tablets act instead by reducing the absorption of dietary fats. As such they can cause flatulence, incontinence, oily excrements and fecal urgency, especially after taking particularly lipid-rich meals.

In 2015, the European medicines agency approved the use of two new "anti-obesity" drugs to promote weight management in adult patients.

The two drugs - respectively based on the active ingredients naltrexone and bupropion (Mysimba ®) and liraglutide (Saxend ®) - act at the level of brain centers that control satiety, reducing appetite and the amount of food consumed by patients.

Mysimba ® is available as prolonged release tablets for oral intake, while Saxenda ® requires subcutaneous injection in the thigh, upper arm or abdomen.

There are also numerous other drugs that have an important weight loss effect but are not approved for this purpose. These include thyroid hormones (see eutirox ), acarbose, fendimetrazine and its analogues: phentermine, benzfetamine, diethylpropion and mazindol . Among the drugs banned for the rather serious side effects, but alas still available on the black market, for completeness I remember amphetamines ( fenfluramine and dexfenfluramine ) and phenylpropanolamine. All the slimming tablets mentioned in this paragraph have precise side effects and contraindications; as such they must necessarily be taken under strict medical supervision, according to the recommended methods. Among the general risks of abuse there are thyroid dysfunctions, pulmonary complications (primary pulmonary hypertension), cerebral (stroke) and cardiac (histological changes of the valves of the heart, hypertension).

The range of weight loss supplements is even wider. Their effect is undoubtedly inferior to that of the aforementioned drugs, but also the risk of side effects is generally contained. Following is a brief overview of the most popular weight loss tablets marketed as supplements.

ANORESSISERS AND MODULATORS OF INTESTINAL ABSORPTION : dietary fiber supplements, taken together with generous amounts of water, swell inside the stomach, promoting a sense of satiety. Moreover, they act in the intestine modulating and reducing the absorption of fats and carbohydrates. These weight loss supplements include: bran, guar and guar gum, karaya gum, psyllium, psyllium seeds, agar agar, glucomannan, pectin, Konjac flour, algina and alginic acid, carrageenan. Fiber supplements should not be taken before going to bed, as they can cause oesophageal obstructions with consequent nocturnal asphyxiation by rapidly swelling. In general, its use is not recommended in the presence of intestinal obstructions or inflammation, pyloric stenosis, megacolon or megaretto, marked meteorism and functional dyspepsia. It should also be remembered that the massive intake of bran for therapeutic purposes significantly reduces the intestinal absorption of calcium, zinc, magnesium and phosphorus, all essential elements for the organism, especially during growth, in the elderly, in debilitated subjects and in women pregnant. In this case, it is essential to supplement the diet with vitamin and mineral supplements.

THERMOGENIC AND NERVINE FOODS : they reproduce the action of catecholamines favoring their release and / or stimulating their receptors; this causes an increase in the metabolic activity of the organism. This category includes tablets and various extracts of bitter orange (unripe fruit), cocoa, coffee (seeds), cguaranà (seeds), maté (leaves), ephedra (aerial parts), black tea (leaves), green tea ( leaves) and yohimbe (bark of the stem). Excessive use of these herbs for weight loss is accompanied by well known and dangerous adverse effects, of a psychic nature (agitation, nervousness, anxiety, insomnia) and cardiovascular (palpitations, tachycardia and hypertension).

SEAWEED SUPPLEMENTS : they are a generous source of iodine - an essential mineral for the synthesis of thyroid hormones - and soluble fibers, such as alginates. The tablets containing seaweed extracts (marine oak (fucus) and other algae) can therefore promote weight loss by supporting the body's metabolic activity and carrying out an anorectic effect. Overdose can cause hyperthyroidism, thyroid dysfunction, tremors, irritability, hot flushes, tachycardia, insomnia and arterial hypertension.
