Indications EUTOPIC ® Gentamicin

EUTOPIC ® is indicated for the treatment of skin infections caused by bacteria sensitive to gentamicin.

Topical use of gentamicin has been shown to be effective in the treatment of pyoderma, folliculitis, furunculosis, microbial eczema, dermatitis, infections due to traumatic injuries and burns and acne and pustular psoriasis.

EUTOPIC ® Gentamicin action mechanism

Gentamicin, the active ingredient of EUTOPIC ®, is a molecule with natural antibiotic activity, isolated for the first time from Micromonospora purpurea.

Classified among aminoglycosides, gentamicin applied to the skin permeates the bacterial wall through hydrophilic protein channels reaching the cytoplasm of the microorganism, where by binding the ribosomal subunit 30S, it prematurely interrupts the peptide chain synesis, inhibiting bacterial protein synthesis.

This activity, characterized by indirect lesions affecting the membranes, seems to favor the entry of new antibiotic into the cell, thus enhancing the dose-dependent microbicidal activity.

After its activity, with a half-life of between 1 and 4 hours, gentamicin is mainly excreted by glomerular filtration.

Despite the excellent efficacy of action, more intense for Gram positive microorganisms, the inadequate use of this antibiotic has facilitated the diffusion of resistant microbial strains, able to reduce the therapeutic activity of the antibiotic, decreasing its cell permeability, changing the ribosomal target proteins, and expressing enzymes on the plasma membrane capable of inactivating the active ingredient before it performs its microbicidal action.

As is known, the diabetic patient is at great risk of skin and soft tissue infections, especially near the lower limbs where the venous and lymphatic system is strongly compromised. In this study the topical use of gentamicin-collagen was effective in improving the clinical conditions of diabetic patients suffering from infectious lesions of the foot.

Very interesting case report that reports the onset of infections in patients undergoing peritoneal dialysis sustained by bacteria resistant to gentamicin. These works highlight the importance of using well-formulated antibiotic therapies while avoiding the inappropriate use of these drugs.

Interesting work that evaluates the efficacy of topical gentamicin therapy in contrasting peritoneal infections in patients on peritoneal dialysis. The data show a significant decrease in infections due to Staphylococcus aureus.

Method of use and dosage

EUTOPIC ®

Topical cream with 0.1% gentamicin sulphate.

It is advisable to apply a correct amount of cream on the affected area from the infective process 3-4 times a day until the initial regression of symptoms.

Therapy can be prolonged by continuing to apply the cream 1 or 2 times a day until the lesions have completely disappeared.

Warnings EUTOPIC ® Gentamicin

EUTOPIC ® therapy must be preceded by a medical consultation in order to verify the prescription appropriateness reducing the possible side effects deriving from the incorrect use of the antibiotic.

An inadequate use of these drugs could in fact easily increase the risk of onset of microbial strains resistant to the common antibiotic therapy, therefore dangerous from the clinical point of view.

Prolonged use of EUTOPIC ® may also increase the risk of sensitization to gentamicin, exposing the patient to possible hypersensitivity reactions.

Given the possible side effects of gentamicin, the use of EUTOPIC ® should be carried out with particular care in patients suffering from musculoskeletal disorders, the auditory-vestibular apparatus and the urinary tract.

For this reason, periodic monitoring of the patient's health status with gentamicin would be advisable.

PREGNANCY AND BREASTFEEDING

The use of gentamicin in pregnancy has recently been associated with the appearance of irreversible bilateral congenital deafness in newborns exposed to the antibiotic, thus allowing to extend the contraindications to the use of EUTOPIC ® also to the gestational period and to breastfeeding.

Interactions

The low systemic absorption that characterizes the use of EUTOPIC ® allows to minimize the possible drug interactions able to alter the pharmacokinetic characteristics of gentamicin.

Contraindications EUTOPIC ® Gentamicin

The use of EUTOPIC ® is contraindicated in case of hypersensitivity to the active ingredient, to structurally related drugs and to the relative excipients.

Undesirable effects - Side effects

Topical use of gentamicin is usually well tolerated and has no clinically relevant side effects.

Sometimes it is possible to observe on the cutaneous region concerned the appearance of erythema accompanied by pruritus of medium severity, such as not to require the suspension of the pharmacological therapy in progress.
