Indications BAYPRESS ® Nitrendipine

BAYPRESS ® is a drug indicated for the treatment of high blood pressure.

Mechanism of action BAYPRESS ® Nitrendipine

Nitrendipine - a molecule belonging to the diidrdopiridine family, as well as an active ingredient of BAYPRESS ®, is rapidly absorbed from the gastrointestinal tract following oral administration, guaranteeing that the plasma peak is reached on average 2 hours after intake. Despite being absorbed for about 90% of the entire dose, the bioavailable portion - therefore biologically effective - is represented by 20/30% of that absorbed, given a first-pass hepatic metabolism that oxidizes it into inactive metabolites. The remaining portion, mainly linked to plasma proteins, can be placed on the surface of smooth muscle cells of vascular structures, reducing the activity of calcium channels and preventing the incoming flow of this mineral.

The increase in intracellular calcium concentrations represents the key event in the mechanisms of cellular contraction, as the mineral - linked to specific binders - is able to rearrange the actinic and myosin proteins allowing the fibrocellula to contract.

The molecular action of nitrendipine takes the form of a decrease in peripheral resistance, with an important antihypertensive effect which is generally followed by a transient and modest phase of adaptation, characterized by an increase in heart rate.

The antihypertensive effect appears to be enhanced, at least in part, by the natriuretic action that this drug performs especially in the initial stages of treatment.

The half-life of nitrendipine is about 8/12 hours, after which it is metabolised in the liver and eliminated mainly in the kidney.

Studies carried out and clinical efficacy

1.NITRENDIPINA, NOT ONLY AN ANTI-SENSITIVE

It is known that calcium antagonists, like other antihypertensives such as diuretics, can guarantee a reduction in morbidity and mortality due to cardiovascular accidents. However, several studies are also clarifying the role - probably unique among antihypertensive drugs - of calcium antagonists in the reduction of oxidative damage, in the prevention of LDL oxidation and in the prevention of atherosclerotic diseases. If this effect seems to have already been confirmed for some active ingredients belonging to this pharmaceutical category, the preventive potential of nitrendipine still remains to be clarified.

2. ASSOCIATED THERAPY IN SLIGHT HYPERTENSION

The combination of nitrendipine with other antihypertensive drugs has also proved particularly effective in controlling blood pressure in patients with mild to moderate short-term hypertension. In fact, 12 weeks of treatment with nitrendipine / enalapril guaranteed not only good blood pressure control, but also a reduction in the side symptoms associated with monotherapy.

3. NITRENIDIPINE IN THERAPY COMBINED IN THE TREATMENT OF HYPERTENSION

This study - conducted on 5292 patients suffering from hypertension, who were given nitrendipine in synergy with hydrochlorothiazide for about 15 months - demonstrates how this treatment can guarantee a blood pressure lowering of 16 mmHg for systolic and 9.4 mmHg for diastolic, without clinically relevant side effects.

Method of use and dosage

BAYPRESS ® 20 mg nitrendipine tablets: for the treatment of high blood pressure it is recommended to take one tablet a day, possibly in the morning after breakfast, or half a tablet twice a day, for a total of 20 mg.

The recommended dosage, however, may be subject to variations, indicated by the doctor based on the patient's physiopathological conditions, the possible concomitant administration of other drugs or in the case of pathologies of hepatic interest.

Warnings BAYPRESS ® Nitrendipine

Given the hepatic metabolism of nitrendipine, the administration of this drug in patients with various types of liver disease should be carried out with particular care and starting from the minimum possible dosages.

Furthermore, the high susceptibility of cytochromic enzymes responsible for the oxidation of this active ingredient requires particular attention in the management of drug therapy, with frequent monitoring of blood pressure, such as to avoid hypotensive crises and at the same time guarantee rapid adaptation of the dose.

Possible side effects, in particular dizziness, drowsiness and dizziness, and high individual variability in therapeutic response, could reduce the patient's perceptive and reactive capacities, making it dangerous to use machinery and vehicles.

PREGNANCY AND BREASTFEEDING

BAYPRESS ® is contraindicated in pregnancy, given the correlation demonstrated in the literature between calcium antagonist intake and fetal malformations and embryotoxicity, effects probably due to the alteration of the normal fetal flow.

Nitrendipine is secreted in breast milk; therefore it is not recommended to take it during breastfeeding, whereas if its use is necessary, breastfeeding should be stopped.

Interactions

Given the high susceptibility of enzymes responsible for the metabolism of nitrendipine (cytochromes) to the action of different molecules, BAYPRESS ® can interact with:

  1. Rifampicin, phenytoin, phenobarbital, determining an acceleration of its metabolism, with consequent reduction of therapeutic efficacy;
  2. Macrolide antibiotics such as erythromycin, which - on the contrary - are able to determine an inhibition of cytochromes, increasing plasma levels and therapeutic efficacy of the active ingredient;
  3. Antidepressants such as fluoxetine, and grapefruit juice, with increased plasma levels of nitrendipine;
  4. Other antihypertensive drugs, leading to a marked hypotensive effect.

To these we should also add all the other cytochrome inducers and inhibitors, including alcohol, for which, however, the possible interaction has not yet been tested, but which could interfere with the normal pharmacokinetic properties of nitrendipine.

Contraindications BAYPRESS ® Nitrendipine

BAYPRESS ® is contraindicated in case of hypersensitivity to one of its components, in case of angina pectoris, recent myocardial infarction and during the entire period of pregnancy and lactation.

Undesirable effects - Side effects

The most common adverse reactions to BAYPRESS ® are characterized by anxiety reactions, headache, palpitation, edema, vasodilation and asthenia. The incidence of myalgia, increase in transaminases, tachycardia and angina appears rarer. Rare are also possible hypersensitivity reactions to one of the components of BAYPRESS ®
